Sunfrail Project comes to the end with the Final Conference in Bologna on February 7th.

The SUNFRAIL Final Conference will take place in Bologna and it will be dedicated to present the main results obtained by the 11 partners during the 34 months of project implementation. So far, the following achievements have been reached: mapping of good practices, design of Sunfrail Model of Care, design of Sunfrail Tool Conceptual Frame, development of the Sunfrail Tool for the early identification of frailty and multimorbidity and the design of the Sunfrail Tool for Human Resources.

Moreover, the final conference will be used to propose recommendations for the replicability and sustainability of the model and of the tools identified. Furthermore, the conference focuses on programs and initiatives in the field of frailty and multimorbidity. On the other hand, the Final Conference will include panel and plenary discussions with high level European and international speakers from the European Commission and national and regional health departments.
